In Domino, when recording user activity for a database, the user name or server name is sometimes not being recorded.
Step to reproduce this issue:
1. Open the Properties box (File, Database, Properties) of a particular database and click the "i" information tab.
2. Click the User Activity/User Detail button and enable "Record Activity".
3. Open some documents in the database with various IDs.
4. Open the User Activity box again and note that the log does not include the user name or server name that accessed the database. The result is as follows:

Solution
This issue has been reported to Quality Engineering as SPR# LMEF4DKPEF; however, it was determined not to be a software problem. Notes is functioning as designed.
The User Activity log will be populated without an associated User Name when certain administrative actions take place, such as flushing a database or where secondary databases are opened, like the desktop file, cache.dsk, personal NAB, etc. This eliminates database overhead when it is not needed. An example is when the Notes Index Facility opens a database with full access so a collection's view note can be updated when the collection and container items change.
